Who can enroll
In Utah, injecting a neurotoxin for cosmetic purposes is a cosmetic medical procedure — the practice of medicine. Only licensed medical and nursing professionals may perform it, and only under a prescriber's order. We verify every license with DOPL (or your state board) before confirming a seat.
Compact-state RNs are welcome. Out-of-state NP, PA or MD licensees: ask us about a Utah temporary license before enrolling.
Injects under the prescriber's order. Our most common student.
Independent prescriber in Utah. Can order, inject and supervise.
Under a collaborative practice agreement with a physician.
Dentists: oral and maxillofacial region, within dental scope.

Two days, start to finish.
Two consecutive weekdays, 9 am to 6 pm, with supervised hands-on practice on both days in the treatment rooms of Forever Beauty Med Spa, West Jordan. No prior injecting experience required. Dates are set with each small group — reserve and we'll coordinate the next available two-day block with you.

- Foundations. Utah law for injectors; applied facial anatomy and danger zones; toxin pharmacology, reconstitution, dilution and dosing by area; patient assessment, photography and consent; technique for forehead, glabella, crow's feet and brow.
- Supervised practice — reconstitute, map and treat your first model (upper face) under Morgan's direct supervision.
- Refinement and beyond the upper face. Case review of Day 1 results; complications (ptosis, asymmetry, Spock brow) and management; introduction to masseter, lip flip, DAO, bunny lines and platysmal bands with instructor demonstration.
- Supervised practice — two more models, full upper face, with dosing decisions made by you and checked by the instructor.
- Written exam (80% to pass), practical checklist sign-off, certificate of completion (16 contact hours).

Every practice session has two licensed professionals in the room: the RN instructor at your chair and the prescriber who evaluated your model and wrote the order.
Sidney Stoker, RN
Registered nurse since 2021 and an injector from her first year of practice, Sidney treats neurotoxin and filler patients every week at Forever Beauty Med Spa and wrote the spa's injectables consent and charting protocols. She teaches the way she works at the chair: anatomy first, conservative dosing, and a plan for every complication before the first pass.
Morgan Galbraith, APRN
Advanced practice registered nurse and the prescriber behind every injectable treatment at Forever Beauty Med Spa. Morgan evaluates each model before treatment, writes the order, and stays in the treatment area throughout practice — the supervision Utah requires, and the second set of eyes every new injector should want.
One price, everything in the room included
Comparable courses in Utah run a single day for $2,950. Ours gives two full days, six students per group, and every practice session inside a med spa that treats these patients every day — models, product and follow-up included.
- Reserve — $500 deposit and a two-minute form. We verify your license within one business day, send the enrollment agreement by DocuSign, and confirm your two-day block.
- Balance — $1,900 due 14 days before your course. ACH is fee-free; cards add 3%.

Save your seat
Six seats per group. This program is open only to licensed clinicians. Fill in your license details and send the request: we verify the license with the issuing board first, and only then confirm your seat, your two-day block and the $500 deposit.
Before you enroll
When are the dates?
We run small groups of six and set each two-day block with the students who have reserved, usually within four to six weeks. Reserve with the deposit, tell us your preferred timing, and we'll confirm dates with you before the balance is due.
Who supervises the hands-on practice?
Morgan Galbraith, APRN, evaluates each model, writes the treatment order and is physically present in the treatment area throughout, as Utah Code §§ 58-1-505 and 58-1-506 require. Sidney Stoker, RN, teaches technique and works next to you at the chair.
Will I be "certified" to inject after the course?
You receive a Certificate of Completion documenting 16 contact hours and a passed practical checklist. There is no state or national injector certification. What lets you inject is your license plus the supervision or collaboration your state requires — for an RN in Utah, a prescriber's order after a good-faith exam. We spend the first hour of Day 1 on exactly this.
I'm an esthetician or master esthetician. Can I attend?
You cannot inject in Utah, so we don't offer hands-on seats. If you coordinate injectable clients at a spa, ask about a theory-only observer seat for Day 1 morning.
Do the contact hours count toward my license renewal?
Utah RNs can renew with 30 hours of continuing education every two years (or a mix of CE and practice hours). We issue a certificate of contact hours; whether a board accepts it is the board's decision. Ask us if you need a specific provider statement for your state.
Where do the models come from?
They are patients of Forever Beauty Med Spa who choose to be treated in a training session. Every model signs the clinical consent plus a training-session consent, is charted as a patient, and gets a complimentary two-week follow-up. You don't need to bring anyone.
Do I need my own malpractice insurance?
Yes. Bring proof of your professional liability policy, or ask about a per-course policy we can arrange. Most nurses' policies cover training with a small rider.
Is there a payment plan?
The deposit reserves your seat; the balance is due 14 days before. If you'd like to split the balance, ask when you reserve.
